  1. Remember: always download a form, change the title by adding your own name or identifying information and save it on your computer before filling the form out.
  2. The boxes that ask for long answers have limits on the number of words (actually, letters or “characters”) you can write in.  The smaller boxes have a limit of 400 characters, including spaces.  The limit for mid-sized boxes is 700, and for the largest boxes, the limit is 1000 characters with spaces.

<li><span style=”color: #800000;”>Save the form with your own name or other identifying information to your desktop or a file you create.  If you need more than one copy, download/identify separately.</span></li>

<li><span style=”color: #800000;”>For forms that need a signature, print the form, sign it and scan it. Then save the scanned page with the other filled pages of the form.</span></li>
<li><span style=”color: #008000;”>If the form must be sent on to someone else for verification or completion, send with instructions to return the signed form to you (either a scanned copy or by mail.)</span></li>

If you prefer, you may print filled forms, sign and mail them. You may even print out a non-fillable version and complete it by hand.
